Germanium-68/gallium-68 generators
A compact column loaded with a long-lasting parent isotope lets a PET radiopharmacy produce fresh gallium-68, used in scans such as PSMA and DOTATATE PET, on demand without needing a cyclotron.
Overview
The 68Ge/68Ga generator holds parent germanium-68 (half-life 270.8-271 days) fixed on a solid-phase column, commonly TiO2, SiO2, SnO2 or hydrous stannic-oxide-type matrices, and yields daughter gallium-68 (half-life 67.7-68 minutes) by elution with dilute hydrochloric acid (typically 0.05-0.1 M HCl). Because 68Ge has a nine-month half-life, a single generator supplies clinically useful 68Ga for roughly a year, sufficient for hundreds of elutions, without requiring an on-site cyclotron. Regulatory eluate specifications (Ph. Eur. monograph 2464) require 68Ge breakthrough of no more than 0.001% of eluted activity; generators are typically eluted at least once per working day and pre-eluted with 5x4-10 mL washes after breaks of 72-96 hours to keep breakthrough within limits.
Eluate volumes (4-10 mL) are usually too dilute for direct high-specific-activity radiolabelling, so cation- or anion-exchange fractionation/concentration steps are commonly used before adding the eluate to a peptide precursor (for example DOTATATE or PSMA-11) at 90-95°C for 10-20 minutes to form the 68Ga complex. In routine operation, elution yields around 60-85% of theoretical maximum activity have been reported over hundreds of elutions per generator-year, with radiochemical purity of finished 68Ga-DOTATOC/68Ga-PSMA products typically exceeding 95-99%. The approach underlies most 68Ga-DOTATATE (NETSPOT-type kits) and 68Ga-PSMA-11 production worldwide and is complementary to cyclotron-based 68Ga production, which is increasingly used where generator throughput or supply is constrained.
How it works
The nine-month half-life parent germanium-68 is fixed on a solid-phase column (titanium, tin or silicon oxide); eluting the column with dilute hydrochloric acid selectively washes off the freshly formed gallium-68 daughter while germanium-68 stays bound, so the same column keeps supplying fresh gallium-68 for about a year without needing a cyclotron on site.
