PRINCE
Combining the radioactive drug lutetium-PSMA with the immunotherapy drug pembrolizumab produced strong PSA responses in men with advanced, heavily pretreated prostate cancer, with side effects in line with what each drug causes on its own.
Overview
PRINCE was a single-arm, multicentre phase 1b/2 trial led by the Peter MacCallum Cancer Centre, testing whether adding the checkpoint inhibitor pembrolizumab to 177Lu-PSMA-617 could improve on radioligand therapy alone in metastatic castration-resistant prostate cancer, a disease with generally low response to immunotherapy alone. Eligible men had progressed on an androgen receptor pathway inhibitor (prior docetaxel allowed) and had high PSMA expression on PET. Participants received up to six cycles of 177Lu-PSMA-617 every six weeks with pembrolizumab 200 mg every three weeks for up to 24 months. Co-primary endpoints were safety and PSA50 response rate.
Among 37 participants (73% docetaxel-pretreated, 100% ARPI-pretreated), a PSA decline of 50% or more was seen in 76%, and 46% had a PSA decline of 90% or more. In the subset with measurable disease, the objective response rate was 70%. Median overall survival was 20.8 months. Toxicity was consistent with the known profiles of each drug: xerostomia, fatigue and pruritus were common but mostly grade 1-2, while grade 3 immune-related adverse events (colitis, pancreatitis, myasthenia gravis, and others attributable to pembrolizumab) occurred in 30% of patients; there were no grade 4 events or treatment-related deaths.
PRINCE reports encouraging activity and manageable toxicity for the combination, broadly in line with 177Lu-PSMA-617 monotherapy benchmarks from VISION, and the authors conclude that radioligand therapy remains the principal driver of response with pembrolizumab possibly adding benefit in a biologically susceptible subset. It is a single-arm, non-randomised study, so it cannot establish incremental benefit from adding pembrolizumab.
- 76 out of 100 people had their tumour shrink with 177Lu-PSMA-617 (up to 6 cycles) + pembrolizumab (up to 24 months).
- There was no comparison group, so this number cannot tell you how much of the effect is due to the treatment.
- A response rate counts how many people had their tumours shrink by a set amount; shrinking is encouraging but does not by itself show that people live longer or feel better.
- 70 out of 100 people had their tumour shrink with 177Lu-PSMA-617 + pembrolizumab.
- There was no comparison group, so this number cannot tell you how much of the effect is due to the treatment.

- Median 20.8 months with 177Lu-PSMA-617 + pembrolizumab.
- A median is a midpoint: half the people did better than this and half did worse.
- Overall survival counts deaths from any cause, so it is the most direct measure of whether a treatment helps people live longer.
- 30 out of 100 people reached this endpoint with 177Lu-PSMA-617 + pembrolizumab.
- There was no comparison group, so this number cannot tell you how much of the effect is due to the treatment.
- This endpoint is not one of the standard survival or response measures; read it alongside the trial's primary result.
- These results apply to the people the trial enrolled: Metastatic castration-resistant prostate cancer with high PSMA expression, after prior androgen receptor pathway inhibitor (with prior docetaxel allowed); combination therapy trial. People in a different situation may not see the same effect.
- The trial selected people by a biomarker (PSMA); the result should not be assumed for people whose cancer does not have it.
- Only 37 people took part, so the numbers are less certain than in a large trial.
Numbers are from the trial as recorded here; see the source links in the table below. This is orientation, not medical advice: ask your team how closely the trial population matches you.
