Radioligand dosimetry
Measuring, from scans taken after each dose, how much radiation a radioligand actually delivers to the tumour and to kidneys and marrow, so treatment can be personalised.
Overview
Radioligand therapies are mostly given at fixed activities, unlike external radiotherapy which is planned to a dose. Post-treatment SPECT or PET imaging at several time points lets physicists calculate absorbed doses to tumours and organs at risk, and trials are testing dose escalation guided by kidney and marrow dosimetry. European regulation now expects dosimetry to be available, and software and imaging protocols are being standardised to make it routine.
- Gamma camera (rotates)
- Single photons (Tc-99m, Lu-177)
How it works
Serial quantitative imaging after administration measures activity in organs over time; time-integrated activity and dose kernels give absorbed dose per organ, which guides subsequent cycles.
- Personalises activity to each patient
- Explains variable toxicity and response
- Required by European regulation
- Extra imaging visits
- Methodological variation between centres
- Prospective outcome evidence still limited

This document standardises how hospitals measure the actual radiation dose reaching a patient's kidneys and tumours during lutetium-177 treatment, laying the groundwork for shifting from a one-dose-fits-all schedule to treatment doses tailored to each patient's biology.
This paper is a map of the radioactive-cancer-drug field at a pivotal moment, explaining to non-specialists and drug developers alike why these treatments work differently from chemotherapy and what needed to be fixed - supply, dosing science, regulation - before more of them could reach patients.
In the largest reported group of neuroendocrine tumour patients treated with individually dosed radioactive peptide therapy, most people lived several years after treatment, and serious long-term harms such as blood cancers were uncommon but real, occurring in about 1 in 50 patients.
