FAP imaging to stop futile pancreatic surgery
Four in ten pancreatic cancer patients whose scans say the tumour can be removed turn out, once opened up, to have disease that cannot be removed. A scan that lights up the scar tissue around tumour deposits sees more of that hidden disease than the standard one.
Overview
The number that defines this opportunity comes from a Cochrane review: the median pre-test probability of unresectable disease after CT was 41.4%, meaning 41 of 100 participants whose cancer looked resectable on CT were found unresectable at laparotomy. The operation that follows is major surgery with no curative benefit.
FAP imaging attacks that gap directly, because pancreatic ductal adenocarcinoma is a stroma-rich tumour and the tracer reports the fibroblast compartment rather than glucose uptake. A 62-patient prospective study of 18F-FAPI-04 against FDG upgraded the TNM stage in 14 patients, with higher uptake in primary tumours, nodes, liver and peritoneal deposits, and FDG missed one patient with cancer that FAPI found. A PDAC-specific meta-analysis of 7 studies and 322 patients gives pooled sensitivity of 0.99 against 0.84 for FDG with comparable specificity. A 20-patient Korean study found FAPI-74 detected metastatic nodes in 50% of cases against 0% for FDG.
The gap in the evidence is precise and worth stating: no trial has yet reported that FAPI-based selection reduces the non-curative laparotomy rate. The completed SOFIE phase 2 (NCT05262855, 63 patients) measured sensitivity against histopathology, not operations avoided. On the therapy side the picture is discouraging — FRONTIER was terminated early for rapid clearance and low tumour doses, and across eight compassionate-use patients the predominant outcome was radiological progression — which strengthens the case for treating this as a diagnostic opportunity rather than a theranostic one.
No validated test tells a pancreatic tumour board, before operating, whether a CT-resectable tumour hides peritoneal or small liver metastases, and 41.4% of CT-resectable patients are found unresectable at laparotomy.
- FAP is expressed in wound healing, pancreatitis and other fibrotic conditions, so specificity in a chronically inflamed pancreas is the obvious failure mode and needs its own dataset.
- Detection superiority does not automatically translate into fewer futile operations; a surgeon will not cancel an operation on an unbiopsied peritoneal signal without prospective evidence.
- The therapeutic side of FAP is going badly, with one terminated trial and mostly progression in compassionate use, so a theranostic pitch would be weak.
- Publication of the SOFIE phase 2 results and whether any follow-on trial adopts avoided laparotomy as an endpoint.
- Specificity data in chronic pancreatitis and post-treatment fibrosis, which is where this agent will either hold up or fail.
- Whether FAP-directed therapy recovers after FRONTIER, which determines if a companion use ever exists.
The five domains of a radiopharmaceutical due-diligence review, adapted from the Fryback-Thornbury hierarchy of diagnostic efficacy. Each carries the state of the public evidence and the fact behind it, not a score out of five: a single total hides the reason and stops telling candidates apart. Domains marked as carrying most weight are the ones that decide the case at this asset's stage.
Pancreatic ductal adenocarcinoma is stroma-rich and fibroblast activation protein is expressed in the tumour stroma rather than the tumour cells, which is why uptake exceeds FDG in primary tumours, nodes and peritoneal deposits.
A completed prospective multicentre phase 2 of 63 patients with histopathology truth standard, a 62-patient prospective head-to-head against FDG, a 20-patient resectability cohort, and a PDAC-specific meta-analysis of 7 studies and 322 patients.
Pooled sensitivity 0.99 (0.97 to 1.00) for FAPI against 0.84 (0.70 to 0.92) for FDG with comparable specificity across 322 patients; stage upgraded in 14 of 62 patients in a prospective comparison.
Diagnostic FAPI agents have been used across many published cohorts without reported safety signals; on the therapeutic side whole-body effective dose for 177Lu-FAPI-XT was 0.039 plus or minus 0.013 Sv/GBq.
No FAP imaging agent is approved anywhere, and no trial has yet used a management endpoint such as avoided laparotomy, which is the claim this opportunity actually needs.
Red, amber or green on each review category, each with its reason, so an obvious no-go takes seconds rather than a meeting.
Works with both gallium-68 and fluorine-18 constructs, same-day imaging, and repeatedly reproduced across independent centres.
A 41.4% futile-laparotomy rate after CT staging is a large, quantified, guideline-recognised failure of current imaging.
The FAPI chemical space is crowded with FAPI-04, FAPI-46, FAPI-74, FAP-2286 and numerous derivatives across multiple sponsors and academic groups.
Standalone: the value is surgery avoided, which needs no drug approval, and a FAP-directed therapy would be an additional rather than a necessary use.
Gallium-68 and fluorine-18 both established; the therapeutic side would need lutetium-177 or an alpha emitter.
A completed multicentre phase 2 exists, Novartis holds the leading therapeutic, and numerous academic groups publish in this space.
