Radio-antibody & radio-ADC
Attaching a radioactive atom to an antibody, so an ADC's targeting is used to deliver radiation instead of chemotherapy.
Overview
90Y-ibritumomab (Zevalin) and 131I-tositumomab proved the concept in lymphoma but were commercially abandoned. Renewed interest: 225Ac-labelled antibodies to PSMA (J591), CD33 (lintuzumab), DLL3, HER2, and 177Lu-labelled antibodies. Long antibody half-life is both a dosimetry advantage and a marrow-toxicity problem; pretargeting and fragment approaches address it.
- Chelated Lu-177 / Ac-225
- Antigen-positive cell
- Radiation, not chemistry
How it works
A chelator-conjugated antibody or fragment carries 177Lu, 225Ac, 212Pb, or 131I.
- Any ADC target becomes a radiation target
- Bystander crossfire independent of payload chemistry
- Marrow dose from circulating antibody
- Manufacturing complexity

Zevalin is an antibody carrying a radioactive isotope that seeks out CD20 on lymphoma cells. It treats follicular lymphoma that has relapsed and is given as a one-off consolidation after chemotherapy.
A cancer-targeting antibody carrying a radioactive form of iodine was used, as a single course, to treat a type of blood cancer called non-Hodgkin lymphoma. The manufacturer stopped selling it in 2014 because too few patients were using it.
Iomab-B is Actinium's radioactive antibody that clears the bone marrow before a transplant for older patients with active relapsed acute myeloid leukaemia; the phase 3 SIERRA trial met its primary endpoint but the FDA asked for another study.
A cancer-targeting antibody carrying a radioactive metal is being tested as a two-dose treatment for advanced prostate cancer, on top of the usual hormone or chemotherapy drugs.
