## TL;DR

Neuroendocrine tumours were the proving ground for the whole idea of theranostics: find a receptor on the tumour, image it with a low-dose radioactive scan, then treat with a matched higher-dose version of the same molecule. That sequence took thirty years to build and is now being extended with more powerful alpha-particle versions.

## Summary

The somatostatin-receptor pathway is where theranostics as a field began. Indium-111 pentetreotide (OctreoScan), approved by the FDA on 2 June 1994, was the first peptide-based radiopharmaceutical approved for any indication, imaging somatostatin receptor 2 (SSTR2) on neuroendocrine tumours by gamma-camera scintigraphy. Gallium-68 dotatate (Netspot), approved 1 June 2016, replaced planar scintigraphy with quantitative PET, giving markedly better sensitivity for localising these often small, slow-growing tumours.

The therapeutic half of the pair, lutetium-177 dotatate (Lutathera), was approved by the FDA on 26 January 2018 on the strength of the NETTER-1 trial, becoming the first FDA-approved peptide receptor radionuclide therapy (PRRT) and confirming that the diagnostic-therapeutic pairing translates into a survival and progression-free-survival benefit, not just better pictures. The label was extended to first-line use in higher-grade (grade 2/3) tumours in 2024 on the strength of the NETTER-2 trial (72% reduction in risk of progression or death versus high-dose octreotide), and to adolescent patients (12 years and older) in April 2024, the first FDA-approved radiopharmaceutical for that age group.

The current frontier is alpha-emitting versions of the same pharmacophore for patients whose disease progresses after beta-emitting PRRT: 225Ac-DOTATATE (RYZ101) is in the phase 3 ACTION-1 trial, and 212Pb-DOTAMTATE (AlphaMedix) reported in October 2025 that its phase 2 ALPHAMEDIX-02 trial met all primary efficacy endpoints in both PRRT-naive and PRRT-exposed patients. A further extension under investigation is SSTR2-expressing meningioma, a non-oncological-in-the-classical-sense but SSTR2-driven tumour, where 68Ga-DOTATATE PET has been used since 2015 to delineate disease and 225Ac-DOTATATE (RYZ101) is now in an early-phase trial for progressive or recurrent intracranial meningioma.
